The Vial of Life is designed to speak for you when you can’t speak for yourself. The vial contains important medical information that can assist Delray Beach Fire-Rescue crews in administering appropriate medical care in the event you or your loved one is incapacitated. This medical information includes:
- Name
- Emergency Contact Information
- Insurance Information
- Allergies
- Medications
- Medical History
How does the vial of life work?
Once you have filled out the Vial of Life, place a copy in a small plastic bag provided by Delray Beach Fire-Rescue. Place this bag on the front of your refrigerator with the Delray Beach Fire-Rescue magnet. Then place the Delray Beach Fire-Rescue sticker on your front door. This will alert crews to the presence of the Vial of Life. Crews will send one member to retrieve the Vial of Life while the rest of the crew begins administering care. Having this information readily available is an important and easy way to ensure the best possible care for you and your loved ones.
How do I get the Vial of Life?
You can pick up the entire Vial of Life at our headquarters located at 501 W. Atlantic Ave, Delray Beach FL 33444. To fill out the Vial of Life electronically click here. Once you have printed the electronic Vial of Life come to our headquarters to pick up you bag, magnet, and sticker.
What else should I put in the Vial of Life?
- A copy of your most recent EKG or 12-lead from your doctor’s office
- A copy of your DNR or Living Will if applicable
Do I mail the Vial of Life to the Fire Department?
Do not mail the vial of life to the fire department! This information is needed at your home so that it may be accessed by crews during a medical emergency.
